Comparative Analysis of Reconstruction after Total Gastrectomy



Abstract
As the role of total gastrectomy for gastric cancer increasing, various reconstructive methods are suggested.
In general, it is possible to classify the reconstruction in two major types, Roux-en-Y and interposition.
A comparative analysis according to reconstructive type was made for 28 patients with gas tric cancer treated at the Department of Surgery, College of Medicine, Chosun, University, from september 1989 to August 1992.
@ES The results were summerized as follows:
@EN 1) Reconstructive procedures were Roux-en-Y esophagojejunostomy in 15 cases and jejunal pouch interposition (esophagojejunoduodenostomy) in 13 cases.
2) There was no significant difference in age, sex, location of lesions, and stage between two reconstructive types.
3) Adenocarcinoma was 26 cases(92.9%), among these, 15 cases were poorly differentiated type and sguamous cell carcinoma was 2 cases(7.1%).
4) No anastomotic leakage was found in two reconstructive groups.
5) There was no significant difference in reflux esophagitis between Roux-en-Y esophagojejunostomy and jejunal pouch interposition.
6) Body weight was returned to the level of preoperative weight at 6th postoperative months in two groups, more rapidly in interposition group.
7) No patients died following operation, a operative mortality rate of 0%.
We conclude that Roux-en-Y esophagojejunostomy and jejunal pouch interposition were satisfactory methods of reconstruction after total gastrectomy, more relatively low morbidity and rapid weight gain in interposition group.
Also, we think that Jejunal pouch interposition is expected to increase absorptive and digestive action action because of duodenal passage and large reservior
